Please visit our office at the address below. We are conveniently located on the corner of W. Northwest Highway and Park Blvd, behind Blockbuster Video adjacent to the nail salon.

Appointments: Your appointment is a specific time reserved for you to see the dentist. The appointment allows the dentist to meet your needs and also schedule other equally important patients. If by chance you can't keep an appointment, our office requires 24 hour notice.

Insurance: If you're covered by dental insurance, we'll fill out the necessay papers for you. Be sure to sign the form so we can release your records.

For more extensive treatments, most insurance companies require pretreatment estimates. We'll be happy to supply any information your carrier needs. Please bring your insurance information with you to the office when you come in to see us.

Your carrier may pay all or some of your bill, but remember, we ask you to pay at the time of service for any balances your insurance won't cover.

Payment Policies: We accept cash, personal checks, VISA, MasterCard, American Express and Discover. Payment is expected as services are rendered, unless prior financial agreements have been made with the business manager. We offer interest free payment plans for 3 months, 6 months, and 1 year with approved credit.

We'll give you a reasonable fee schedule for crowns, bridges, dentures or any other major dental work before we begin treatment.

Emergencies: We're ready to handle your dental emergencies. During working hours, call our office and we'll try to bring you in the same day to care for your problem. After hours, please call Dr. Chu at (817) 329-6000

If you can't reach one of our staff, please leave your name and number on the office answering machine and we'll return your call as soon as we can.

   Office Policies

Confidentiality: We'll release medical information only with your consent.

Smoking: As a safeguard to patient health, please do not smoke in our offices.

Reception Area: Please bring only one person to accompany you on your office vist. This courtesy will allow room in our reception area for all our patients. Children 18 and under must be accompanied by a parent or guardian.
